Project overview
Funded by the Federal Ministry of Health (BMG)
The EliPfad project deals with the rehospitalisation of patients with multimorbidity. The aim of the intervention is to prevent the re-hospitalisation of these patients and to support independent and self-determined living in the home environment. The intervention consists of cross-sectoral smart, digital networking and the implementation of case managers. The project will test the effectiveness of the intervention and carry out a process evaluation.
